No matter how much you talk, threaten, or attempt to persuade it, the door won't let you past. There's a table to the right of the door where you can pick up a few Whispering Masks. The key to getting past the Gnarled Door is just having one member of your party wear the Whispering Mask when you do it.

When Auntie Ethel splits into illusions of herself, look for the one with the debuff on it to find the real boss. When Auntie Ethel clones herself to look like Mayrina, use a low-damage grenade to hit them both and reveal the hag.

If you don't kill her you can make a deal with her for a hag's hair buff and you can still convince her to at least release Mayrina. The buff is basically +1 to an ability, not really a huge loss. Feeing everyone by killing her and then taking her loot is the better option imo.

Go to the Garamsythe Waterway. The semi-circular area you enter coming from Lowtown has the sluice gate controls. You can use the key to open / close gates and lower / raise water in different areas of the waterway.

There is currently no way to remove the Illithid Tadpole parasites in Baldur's Gate 3.
